Health insurance companies are required to follow the same format, so you can have  clear and consistent comparison of plans and carriers.  It will summarize the key features of the plan or coverage, such as the covered benefits, cost-sharing provisions, and coverage limitations and exceptions.

SBCs include a summary of key features of the plan (including covered benefits, cost-sharing provisions, limitations, and exclusions). The summary also includes coverage examples for the cost of having a baby with normal delivery and living with controlled Type 2 diabetes.

How do I get my SBC?

You can find your SBC online at http://shophealthinsurancenow.com/informationcenter/ or by calling the membership number on the back of your id card.

When do I get a copy of my SBC?

Health insurance companies sends out the SBCs at a different times. If you have coverage through your employer, you will receive the summary during open enrollment along with other benefit materials. If you individual coverage, you may receive an SBC while shopping for coverage, after applying, or during enrollment.

You can always call customer service  on the back of your ID card to request a copy of the document.
