In 2014, most individuals will be penalized for not having health insurance. In 2014, the penalty will be 1% of annual income or $95, whichever is greater. In 2015, the penalty will be 2% of annual income or $325. In 2016, the penalty will be 2.5 percent of your annual income or $695. An in-network dentist is one contracted with the dental insurance company to provide services to plan members for specific pre-negotiated rates. An out-of-network dentist is not contracted with the insurance company. Typically, if you visit a dentist within the network, the amount you will be responsible for paying will be less than if you go to an out-of-network dentist.

If no doctors records are ordered, the process typically takes 3 to 4 weeks. If doctors records must be obtained, the process takes 4 to 7 weeks since we are dependent on the promptness of the doctor.
